What is PACE?

Our Palliative Advice Centre is available by telephone 24 hours a day, 7 days a week, 365 days a year. 

We are available to: 

  • Adult patients with a life-limiting illness who are registered with a GP in East Cheshire 
  • Their families and/or carers 
  • Health and social care professionals supporting a palliative patient.

How does it work?

As a patient or carer, your call may be a question about how to deal with symptoms, medication or about any practical aspects of your care.

We give advice and help to connect all the people who look after you, for example; your GP, community palliative care team and district nurses. We try to make sure everyone knows about your care and we work together to support you. That way, your care is well-planned and everyone stays informed.

During the daytime – your call will be taken by our specialist Advanced Nurse Practitioners or our Care Coordinator.  

Overnight – your call will be taken by our experienced nurses in the Inpatient Unit at East Cheshire Hospice. 

Keeping Everyone Informed

All calls are talked about in our daily meeting with the hospital and community Palliative Care team. This is to make sure that all the professionals involved in your care are kept informed.  

Details of your call and the advice given are recorded in your medical records, which are shared with your GP.

We understand this can be a difficult time, and we are here for you 24 hours a day, 7 days a week.

Information for Patients and Healthcare Professionals

Patients:

When you call us, our clinical staff will look at your medical records to help give the best advice. If you’re worried about this please tell us during the call, but if we can’t see your records we may not be able to give as much help.   

Healthcare professionals:

If you are a healthcare professional, responsibility remains with the clinician assessing the patient, and detailed prescribing advice will only be given directly to the prescriber.
