The Sunflower Living Well Centre is here to support anyone whose life has been affected by a diagnosis of a life-limiting illness. We understand that such news can come as an enormous shock, not only to patients, but also to families and carers.
You may feel fearful, anxious or isolated, but you’re not alone. At the Sunflower Centre, we offer a range of services designed to support you and your loved ones, every step of the way.
Patients can be referred by any healthcare professional involved in their care. Once referred, our specialist nurses will work with you to create a personal care plan tailored to your needs and preferences.
You will attend the centre weekly for a 12-week programme, during which:
While you’re with us, you’ll have access to a variety of activities and gentle exercise, delivered in a safe and supportive environment.
Our services are provided free of charge, either in groups or on a one-to-one basis – whichever feels right for you.
