Our experienced social workers and support staff provide compassionate guidance and practical advice, helping you navigate issues such as care needs, legal matters, and financial concerns. We’ll connect you with the right resources and services, tailored to your individual situation.

Planning Your Care

Planning ahead can give you and your family peace of mind. For many, this starts with a conversation — one that our health care professionals are here to support.

We believe that everyone deserves the chance to make informed choices about their future. That’s why we offer Advance Care Planning (ACP) — a thoughtful process that helps you consider and communicate your wishes for care, especially as your illness progresses.

Whether you’re being cared for in the Hospice or at home, we’re here to help you explore:

  • What matters most to you in your care
  • Your preferred place of care and death
  • Treatments you may or may not wish to receive
  • Who you would like to speak on your behalf, if you’re unable to do so

Advance Care Planning is about more than medical decisions — it’s about living well, with dignity and confidence in the care you’ll receive.

Working Together for Your Wellbeing

To ensure you receive the best possible support, we work in partnership with a wide range of organisations including:

  • Specialist Palliative Care teams
  • GPs and District Nurses
  • Age UK
  • The British Red Cross
  • Cheshire Carers

These strong local connections help us link you quickly with the services and support you need.
